‘Understanding integrated care and its role in enhancing the value of palliative and end of life care services’
Integrated care is an approach that seeks to better coordinate care and services around people’s needs. The aim
of integrated care is to improve people’s care experiences and outcomes more cost effectively. In this presentation,
Nick will examine the key building blocks for successful approaches to care integration, the evidence that
underpins the theory, and some of the emerging and most promising emerging models of integrated palliative and
end of life care.
