Representing a wide range of organisations as well as clinicians, services, volunteers, educators and advocates, they bring expertise and dedication to the palliative care sector. Their experience shapes our advocacy, informs our programs, and drives sector-wide collaboration. Together, we are building a stronger, more connected palliative care community, one that ensures quality care is accessible when and where it’s needed most.

Connect Care Australia is a nurse-led Supported Independent Living provider specialising in complex care and high-intensity supports. Our clinical governance framework and multidisciplinary team, including Registered Nurses in management and frontline roles, enable us to provide safe, responsive and person-centred support for individuals with progressive, degenerative and life-limiting conditions.
Only Care
Only Care is a registered NDIS provider delivering person-centred daily living, social and community participation, and registered nursing supports across much of Sydney. Their skilled and empathetic support workforce, backed by multiple Registered Nurses in the leadership team, has particular experience supporting people with neurodegenerative, life-limiting and high-intensity support needs.
Palliative Aged Care Outcomes Program (PACOP)
The Palliative Aged Care Outcomes Program (PACOP) is an evidence-based, best practice model which supports aged care homes (ACHs) to provide optimal palliative and end-of-life care for all residents and their families/carers. PACOP returns reports to drive systematic quality improvement in ACHs and across the residential aged care sector with a national benchmarking program. PACOP aims to directly contribute to the overall strengthening and resilience of our aged care system by providing training and support on the use of the PACOP assessment and response framework. PACOP is a national palliative aged care program fully funded by the Australian Government Department of Health, Disability & Ageing with participation freely available to Australian residential aged care homes.
