Community Packages (ComPacks) (contribution of a nominal fee) provide non-clinical packages such as personal hygiene, domestic assistance, meal prep, transport and social support. This package is available for patients up to 6 weeks after leaving a NSW public hospital

 

Out of Hospital – End-of-life/Palliative Care Package (State funded, nil cost to the individual) provide non-clinical care such as personal hygiene and domestic assistance and are available in most Local Heath Districts.  They are available to individuals who are in the deteriorating or terminal phase of a life limiting illness or condition.  EoL packages are available for 6 weeks but can be repeated depending on the assessed needs of the patient.

Additional End of Life Packages provided by Specialist Palliative Care Services.

Within a few NSW Local Health Districts, Specialist Palliative Care Services provide additional end of life packages. The aim of these packages, provided by Specialist Palliative Care Services, is to reduce unplanned and/or avoidable hospital presentations by registered palliative care clients and facilitate discharges from acute inpatient facilities and Palliative Care Units/Wards to enable clients their choice to die at home.

  • Once registered with your local Specialist Palliative Care Service, check with the service to see if they provide any end of life packages.
  • South-Western Sydney Local Health District operate Palliative Care Home Support Packages (PEACH) packages provided by the Specialist Palliative Care Services within this health district.
  • Sydney Local Health District operate Community Palliative Care at End of Life (PaEL) packages across several local government areas provided by Sydney District Nursing, RPA Virtual Hospital.
