ACD / ACP Telephone Prompt Sheet & Communication Guide
This tools was developed in collaboration with Palliative Care NSW and the Palliative Aged Care Network (PACN). The purpose in developing the ACD/ACP Telephone Prompt Sheet & Communication Guide was to avoid non-beneficial transfers to hospital from a RACF, and to assist RACF staff to effectively communicate when there has been a deterioration in a resident’s condition.

End of Life Law Australia – Aged Care
This is an online training program to support health professionals working in residential aged care and home settings. Once the module is completed a certificate of completion is downloadable.
