A new supportive and palliative care indicators tool (SPICT) RED-MAP introduces a 6-step guide to future care planning regarding conversations with individuals who are experiencing deterioration of their health, as well as their families.
RED-MAP helps us prepare, share information, find out what is important, and agree goals and actions for a future health and care plan.
