Caring for someone or planning ahead?
Join us for a conversation about advance care and end-of-life planning free of charge.

Community Conversations is a free, 1.5-hour education session  around advance care and end-of-life planning.

You’ll gain the knowledge and tools to make informed decisions and access the care you need to:

  • Understand what palliative care is and how to access it
  • Start the conversation with loved ones about what matters most to you
  • Explore the local support services available to you and your loved ones (we invite a member from the local health district to provide this information)
  • Navigate end-of-life matters
  • Learn how to start your advance care plan

Everyone of all ages is welcome! Active retirees, if you have ageing parents and need to start thinking about the supports available for them, if you have a serious illness or are currently caring for someone with a serious illness or if you just want to be better informed navigating end-of-life affairs.

Attendees will receive a free resource kit and the confidence to navigate life’s most difficult transitions.

Want to host a free, in-person session? We’re looking for folks across NSW who’d be willing to host a face-to-face session at their workplace or in their community. Palliative Care NSW will travel to you, secure a venue and provide catering. Or, you can join one of the upcoming sessions listed below.

The Community Conversations project is supported by funding from NSW Ministry of Health and our local partners across NSW.
