The Palliative Care Registered Nurse will work as a member of the Specialist Community Palliative Care Team providing patient centred palliative care for people with advanced life limiting illnesses, their families and carers.
The role will provide a combination of direct/shared/consultative care across settings, in liaison with other Far West Local Health District palliative care health professionals, and is expected to function as a member of a multidisciplinary team including primary care, specialist services, community based services, inpatient, aged care and volunteers.
