About the role:
As a Registered Nurse in Palliative Care at Moree Community Health Service, you will provide compassionate, patient-centred care to individuals facing life-limiting illnesses. Working within a holistic care model, you will ensure patients, and their families receive the support and expertise needed to navigate this important stage of life, all within a supportive community environment.
Key responsibilities include:
- Providing outpatient and inpatient support to palliative care clients across Moree, Mungindi, Boggabilla, Toomelah, and Warialda.
- Delivering patient-centred care in close collaboration with a multidisciplinary team, including Occupational Therapists, Social Workers, Dieticians, Oncology specialists, and Community Nursing teams.
- Enhancing the patient and family journey by supporting end-of-life decisions, including advanced care directives and palliation.
- Contributing to improving the appropriateness and effectiveness of palliative care.
