About the Opportunity

We are looking for an experienced Palliative Care Clinical Nurse Consultant (CNC) with the ability to support the provision of specialist palliative care to cohorts with non-cancer and chronic conditions. This position is a district role and the successful candidate will be required to travel and work across all facilities within Sydney Local Health District.

This position will support the development of the new Integrated Ambulatory Palliative and Chronic Care Program; to improve access for patients, families and/or carers with non-cancer and chronic care who have end of life and palliative care needs. This includes, but not limited to, the following:

  • Development and implementation of new ambulatory Models of Care under the Program.
  • Provision of clinical leadership to support rapid assessments, care planning, and care coordination across Specialist Palliative Care and Chronic Care specialties.
  • Involvement in formal processes for evaluation, strategic development and operational planning activities in accordance with the Program outcomes, Specialist Palliative Care Strategic Plan 2024-2029, and other enabling plans.
  • Contribution to the development and delivery of research and education programs.

The Ideal Candidate is someone who has…

  • A current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA) and 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field.
  • Approved postgraduate qualifications relevant to end of life and palliative care or such other qualifications or experience deemed appropriate.
  • A current NSW Driver’s Licence (P1 and P2 acceptable).
