About You

• Do you hold a current unrestricted driver’s license?
• You are a motivated Nurse Practitioner who has advanced clinical leadership skills and can integrate the principles of clinical governance, quality improvement, evidence based best practice and patient centred care.
• You have advanced nursing practice and with a good knowledge of Microsoft Word, Excel, Outlook and eMR.
• You have solid leadership and communication skills, relevant clinical experience and strong problem-solving abilities that will contribute to the position’s success.
• You can lead by example and can build positive, respective relationships across a wide range of stakeholders in multi-discipline teams.

What You’ll be Doing

• The Palliative Care Nurse Practitioner works with other team members in the Gosford Hospital inpatient consult team to support clients and their families through to End-of-Life care and death.
• You will part of a small multi-disciplinary team who works collaboratively to support patients Internal NSW Health Print iworkfornsw Recruitment Agency and families across the Central Coast.
• You will be responsible for facilitating patient centric care and developing processes that ensure family and carer involvement in patient assessment and management strategies including education and support for their ongoing needs and discharge.
• Working hours are 08:00 to 16:30 Thursday and Friday.
