A snapshot of your role

The SLHD End of Life Care Coordinator (EoLCC) is a dynamic position responsible for clinical leadership, safety, quality improvement and education to support the delivery of end of life care across the SLHD. The position will provide expert consultancy to facilitate evidence-based practices and advanced care planning, and delivery of systems to enhance end of life care which are high quality and person-centred.

Ideal candidate

  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) with minimum five (5) years full-time equivalent post-registration experience, with at least three (3) years experience in specialty field in either Palliative Care/Cancer Care/Advanced Nursing or Aged Care.
  • Proven clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework, with a demonstrated commitment to excellence in nursing practice.
  • Demonstrated knowledge and experience in current end of life and advanced care planning issues.
